The Candidate can obtained application form by Cash payment of by Crossed Demand Draft/Bankers Cheque (MICR only) of Rs. 1060/- (Rs. 660/- for SC/ST candidates) of a Nationalized Bank, drawn in favour of "The Director, Institute of Medical Sciences, BHU" payable at Varanasi. From the admission.
The Application form also can be downloaded from the Website and Submit the Complete fill-up Application form along with all required Documents and DD, The Application must be Sent Before the last date of Submission application form to University Admission Office Address.
